Register with the Practice
In order to register with the Practice you will need to permanently (more than 3 months) live in the G13 3xx,G13 4xx or some G14 0xx and G14 9xx see list of postcode area, the receptionist will ask you to fill in a registration form and a health questionnaire, they will ask you to provide photographic identification and proof of address where possible. Once registered they will offer you a registration medical appointment with the Health Care Assistant.
If you prefer a particular type of doctor e.g. female for a female patient and vice versa we will note this and do our best to respect your choice. However not all the doctors in the Practice provide all services and specific doctors may not be immediately available.
We do not discriminate on the grounds of the applicant’s race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

If you are planning to email registration documents to us, we require the following:
and document(s) confirming your address and identity e.g. picture of a utility bill or something similar with your address on it and also an image of a photographic ID confirming your date of birth.
